📌 1. Definition
The Osmotic Fragility Test (OFT) is a hematological test used to assess the resistance of red blood cells (RBCs) to hemolysis when exposed to hypotonic sodium chloride (NaCl) solutions.
📌 2. Principle
When RBCs are placed in a hypotonic solution, water enters the cells by osmosis, causing them to swell and eventually rupture.
➤ Increased fragility = RBCs rupture more easily.
➤ Decreased fragility = RBCs tolerate more hypotonic conditions before rupturing.
━━━━━━━━━━━━━━━━━━━━━━
📌 3. Clinical Significance
🔴 Increased Osmotic Fragility
➤ Classically associated with hereditary spherocytosis.
➤ Spherocytes have a reduced surface-area-to-volume ratio, making them more susceptible to osmotic lysis.
🟢 Decreased Osmotic Fragility
➤ May occur in thalassemia and iron-deficiency anemia.
➤ These RBCs can generally accommodate more water before hemolysis occurs.
━━━━━━━━━━━━━━━━━━━━━━
📌 4. Traditional Reference Values
➤ Hemolysis begins: Approximately 0.45–0.50% NaCl.
➤ Complete hemolysis: Approximately 0.30–0.33% NaCl.
Note: Values vary with the method and laboratory.
━━━━━━━━━━━━━━━━━━━━━━
📌 5. Specimen and Procedure
➤ Specimen: EDTA-anticoagulated whole blood, according to the laboratory protocol.
➤ Prepare tubes containing decreasing concentrations of NaCl.
➤ Add blood and allow the cells to react under specified conditions.
➤ Assess hemolysis visually or spectrophotometrically.
➤ Compare the results with the laboratory reference range and control.
━━━━━━━━━━━━━━━━━━━━━━
📌 6. Important Clinical Findings
In hereditary spherocytosis, laboratory findings may include:
➤ Spherocytes on peripheral blood film.
➤ Increased osmotic fragility.
➤ Increased reticulocyte count.
➤ Increased unconjugated bilirubin.
➤ Usually negative direct antiglobulin test (DAT).
Modern practice: The eosin-5-maleimide (EMA) binding test by flow cytometry is often preferred when available.

📌 COC & EXIT EXAM PEARL
❓ Which condition is classically associated with increased osmotic fragility?
✅ Answer: Hereditary spherocytosis.
🧠 Remember:
Spherocytes have less membrane surface area relative to their volume, so they rupture more readily when exposed to hypotonic solutions.

📌 Introduction
Red blood cell (RBC) morphology is the examination of the size, shape, color, and inclusions of red blood cells under a microscope using a stained peripheral blood film.
It helps identify abnormal RBC findings and supports the diagnosis of different types of anemia and other hematological disorders.
━━━━━━━━━━━━━━━━━━━━━━
📌 Learning Objectives
After studying this topic, students should be able to:
➤ Explain the principles of RBC morphology assessment.
➤ Describe the morphology of normal red blood cells.
➤ Identify and describe abnormal RBC shapes and forms.
➤ Correlate RBC morphological changes with clinical conditions.
➤ Perform RBC morphology assessment using a stained blood film.
➤ Apply appropriate reporting systems for RBC morphology findings.
━━━━━━━━━━━━━━━━━━━━━━
📌 Normal RBC Morphology
➤ Shape: Round, biconcave discs.
➤ Size: Approximately 7–8 µm in diameter.
➤ Color: Pink with central pallor occupying about one-third of the cell diameter.
➤ Nucleus: Absent in mature RBCs.
━━━━━━━━━━━━━━━━━━━━━━
📌 Major RBC Morphological Abnormalities
➤ Anisocytosis — variation in RBC size.
➤ Poikilocytosis — variation in RBC shape.
➤ Microcytes — abnormally small RBCs.
➤ Macrocytes — abnormally large RBCs.
➤ Hypochromia — increased central pallor due to reduced hemoglobin.
➤ Spherocytes — round RBCs with reduced or absent central pallor.
➤ Target cells — RBCs with a bull's-eye appearance.
➤ Sickle cells — crescent-shaped RBCs.
➤ Schistocytes — fragmented RBCs.
━━━━━━━━━━━━━━━━━━━━━━
📌 Clinical Importance
RBC morphology assessment helps in the investigation of:
➤ Iron-deficiency anemia.
➤ Megaloblastic anemia.
➤ Thalassemia.
➤ Sickle cell disease.
➤ Hemolytic anemia.
➤ Other hematological disorders.
━━━━━━━━━━━━━━━━━━━━━━
📌 Key Laboratory Practice
Always assess RBC morphology in the appropriate monolayer of a well-prepared, properly stained peripheral blood film. Correlate findings with the CBC results and the patient's clinical information.
Remember: RBC morphology supports diagnosis but should not be interpreted in isolation.
